PUT THE IMMUNIZATIONS BACK ON THE P.A.R.
EXTRA PAPERWORK IS INVOLVED WHEN CHECKING OUT A STUDENT. NOW HAVE TO PRINT YET 1/2 MORE PAGES TO GIVE TO THE STUDENT/PARENT. CONVENIENT WHEN ENROLLING A STUDENT. IF IT'S NOT BROKE, STOP TRYING TO FIX IT. WE CLERICAL ARE ON MISIS EVERYDAY 6-7HRS, 5 DAYS A WEEK. ISN'T THE IDEA TO MAKE THINGS A LITTLE EASIER FOR THE STAFF? JUST AN IDEA.

Request was made by Nursing Services. Also, Student Transfer Form only needs to be generated for students leaving the district. Immunization info does not need to be provided to withdrawing students. The parent will need to present immunization card when enrolling in the new district. For students continuing in LAUSD, the data is available in MiSiS.
