PAR (Student Transfer Form) put the the immunization info back on.
The immunization info on the PAR was very useful and saved a lot of time in the attendance and counseling offices. It was recently removed. Please put it back on the report. It's a waste of paper and cuts down on office efficiency to not include the immunization information on the PAR.

Request was made by Nursing Services. Also, Student Transfer form only needs to be generated for students leaving the district. Immunization info does not need to be provided to withdrawing students. The parent will need to present immunization card when enrolling in the new district. For students continuing in LAUSD, the data is available in MiSiS.
